Paula had worked at Ashcroft before the acquisition.
Eleven years.
Good nurse.
Strong evaluations.
No criminal history.
After the takeover, staffing cuts hit.
Nurses were told to:
document risk carefully,
avoid premature discharge,
and escalate patient resistance.
Reasonable on paper.
Then came pressure.
Managers asked:
“Can you support continued stay?”
Paula learned what answers were expected.
She did not invent hallucinations.
Did not fabricate suicide attempts.
But she began writing ambiguous behavior in the most restrictive interpretation.
Patient angry about discharge.
Became:
poor impulse control.
Patient challenged billing.
Became:
fixation, agitation.
Patient asked to call lawyer.
Became:
escalating behavior.
Small wording choices.
Consequences grew.
Paula told herself:
Doctors decide.
Not me.
